Register With The Practice?

Patients will be registered with ‘The Practice’ and not with a named doctor. When patients seek to register with the practice, we do not discriminate on the grounds of race, gender, social class, age, religion, sexual orientation, appearance, disability or medical condition.

All newly registered patients are requested to complete a questionnaire and those aged over five years to MAKE AN APPOINTMENT WITH THE HEALTHCARE ASSISTANT FOR A BASIC HEALTH CHECK.

How Do I....
Change My Details?

Please tell us as soon as possible if you or your family change your name, address or telephone number.

You should contact reception and request a Change of Details form. This will need to be completed and returned to the receptionist as soon as possible.
