Register With The Practice?

Patients are now registered with ‘The Practice’ and not with a named doctor. The old ‘personalised list’ system is now defunct. However, you may see any doctor you choose once you have registered. When patients seek to register with the practice, we do not discriminate on the grounds of race, gender, social class, age, religion, sexual orientation, appearance, disability or medical condition.

All newly registered patients are requested to complete a questionnaire and those aged over five years to MAKE AN APPOINTMENT WITH THE HEALTHCARE ASSISTANT FOR A BASIC HEALTH CHECK.

How Do I....
Change My Details?

Please tell us as soon as possible if you or your family change your name, address or telephone number.

Please contact Reception and request a Change of Details form. This will need to be completed and returned to the Receptionist as soon as possible.
